What to Expect on Your First Visit

We ask that you arrive 10-15 minutes before your scheduled appointment window so that our team can assist you with insurance filing, completing any necessary paperwork, and taking care of payment. 

What to Bring with You

  • A driver’s license or copy of your identification

  • Insurance cards and pertinent information

  • A list of prescriptions and supplements you are currently taking

  • A method of payment

  • New patient forms (recommended)

Payment 

Payment is required at the time of service. Acceptable forms of payment include cash, credit card, and debit card. Please note that checks are not accepted. 

Insurance and Billing

Our practice accepts Medicare patients and also works with a number of insurance providers. 

New Patient Forms

We ask that you save time by filling out your new patient forms and bringing them with you on your first visit. This will help our team reduce the amount of paperwork transferred between you and our staff while also speeding up the process of getting you checked in and seen by a physician as quickly as possible.
